Mid-row passenger side seat belt guide/cover broken jamming the seat belt preventing it to retract. Failure repeated three to four times after each separate repairs. I'm concern for my 7-year old daughter who may not be properly restrainted after she leans forward and the seat belt does not retract.

The mechanism that retracts seat belt, has a ratchet option for car seat usage. During normal driving, the mechanism turns into ratchet mode, tightening and trapping child in belt. It left red burn on childs stomach. Soffocation could have resulted in death had the child been sleeping during this trip. I am disappointed with toyota to only have one tether restraint in the 2nd row. So if you have two small children in car seats they both can't be tethered in the 2nd row. Also the placement of this 2nd row tether is terrible! It interfears with the person in the 3rd row, their feet get tangled in the tether. Not safe!!! Dt
